Survival Analysis of Aging in Place for Long-Term Care Beneficiaries: Focusing on the Impact of Dementia

dc.description.abstract본 연구는 2008-2019년 노인코호트자료(2.0)를 활용하여 10.4만 명의 장기요양인정등급을 받은 자, 특히 치매 노인에 대한 Aging in Place(AIP) 생존분석을 통하여 지역사회 계속거주 생존율과 요양시설 및 요양병원 이탈에 영향을 미치는 요인을 실증적으로 분석하였다. 생존분석 결과, 치매 집단은 비치매 집단에 비해 재가서비스 이용비율이 9.1%p 낮았으며, 독거가 아닌 동거의 경우, 주수발자가 없는 경우에 AIP 실현 가능성이 감소하였다. Kaplan-Meier 분석에서는 치매가 재가 생존기간을 단축시키며, 특히 가족 주수발자가 없을 때 재가 유지 확률이 급격히 감소하는 것으로 나타났다. COX 비례위험 회귀분석에서는 치매가 다른 요인을 통제한 상태에서 요양시설 및 병원으로의 이탈 확률을 38.7% 유의미하게 증가시키는 것으로 확인되었다. 본 연구는 치매가 재가 거주 지속에 장애 요소로 작용하며, 동거 여부 및 가족수발 자원의 유무에 따라 시설화가 가속화됨을 강조하고, 치매를 가진 장기요양 인정자 특성에 맞춘 AIP 지원 정책 마련의 필요성을 제안하였다.
dc.description.abstractThis study aimed to empirically analyze the factors influencing the survival rate of community living and transitions from nursing homes or nursing hospitals, using Aging in Place (AIP) survival analysis. Data were drawn from the National Health Insurance Service’s Elderly Cohort (version 2.0) from 2008 to 2019, encompassing approximately 104,000 long-term care beneficiaries. Survival analysis results revealed that the dementia group had a 9.1 percentage-point lower rate of community-based service utilization compared to the non-dementia group. Furthermore, AIP feasibility was lower for those who were cohabiting rather than living alone, and for those without a primary caregiver. Kaplan-Meier analysis showed that dementia shortened the community survival period, with a particularly sharp decline in retention probability when no family caregiver was available. Cox proportional hazards analysis further revealed that dementia significantly increased the likelihood of transitioning to a nursing facility or hospital by 38.7%, even after controlling for other factors. This study underscores that dementia acts as a significant barrier to sustaining community-based living and accelerates institutionalization, particularly for those living alone and in the absence of family caregiving resources, highlighting the need for AIP support policies tailored to the unique characteristics of long-term care beneficiaries with dementia.
