Factors Influencing the Intention to Use Primary Care Physicians Among People with Disabilities: Focusing on People with Severe Disabilities

dc.description.abstract본 연구는 장애 정도가 심한 장애인(이하 ‘심한 장애인’)의 건강주치의 이용 의사에 미치는 영향 요인을 규명하여 장애인 건강주치의 활성화를 위한 근거를 마련하고자 하였다. 연구 대상은 2020년 장애인실태조사 원시 자료 중 등록장애인 7,025명 가운데 심한 장애인 3,157명이며, Andersen 행동모형을 토대로 분석을 수행하였다. 소인성, 가능, 필요요인과의 관련성은 카이제곱 검정으로, 영향요인은 건강주치의 이용 의사를 종속변수로 하여 다중 로지스틱 회귀분석을 시행하였다. 분석 결과, 건강주치의 이용 의사가 있는 비율은 49.57%(1,565명)였으며, 영향요인으로는 소인성요인에서 연령, 가능요인에서 거주지역, 보건의료서비스 이용 여부, 복지사업 정보 취득처, 코로나19로 인한 어려움, 건강검진 여부, 건강주치의 인지도였고, 필요요인에서 주관적 건강상태, 만성질환 유무, 장애유형, 일상생활 도움 필요정도, 우울 상태가 통계적으로 유의하였다. 특히 복지사업 정보 취득처와 건강주치의 인지도는 제도 활성화의 핵심 요인으로 나타났으며, 단순 정보매체 홍보보다 사회복지·행정·교육기관과 장애인단체·종교단체를 통한 직접적 정보 제공이 더 효과적인 것으로 확인되었다. 이러한 결과는 정부와 관련 기관이 건강주치의 인지도를 높이고 지역별 서비스 격차를 해소하는 맞춤형 홍보 전략을 수립함으로써, 제도의 전국적 확대와 장애인의 건강권 증진에 기여할 수 있음을 시사한다.
dc.description.abstractThis study aimed to identify the factors influencing the intention to use primary care physicians (PCPs) among people with severe disabilities (hereafter "severe disabilities") and to provide evidence for strengthening the PCP program for people with disabilities. Data were drawn from the 2020 National Survey on Persons with Disabilities, and a total of 3,157 individuals with severe disabilities were analyzed. Andersen’s Behavioral Model was used as the theoretical framework. Chi-square tests were performed to assess associations between predisposing, enabling, and need factors and the intention to use a PCP. Multiple logistic regression analysis was conducted to identify significant predictors. The proportion of those intending to use a PCP was 49.6%. Significant predictors included age (predisposing factor); region, healthcare service use, sources of welfare information, experiences of difficulties due to COVID-19, health check-ups, and awareness of the PCP system (enabling factors); and self-rated health status, chronic disease, type of disability, need for assistance in daily living, and depressive symptoms (need factors). Notably, sources of welfare information and awareness of the PCP system emerged as key factors. Direct information from welfare, administrative, or religious organizations was more effective than mass media in promoting intention to use the service. These findings highlight the need for targeted outreach and awareness strategies to reduce regional disparities and enhance the nationwide implementation of the PCP system for people with disabilities.
