The Effect of Perceived Human Rights-Friendly Service Environment on the Recovery of Individuals with Psychosocial Disabilities: Verifying the Mediating Effect of Activity and Participation

dc.description.abstract인권친화적 서비스 환경은 정신장애인 당사자의 자기결정에 기반하여 조성되어야 하므로, 국내 입원병원과 지역사회 정신건강시설에서도 인권친화적 서비스 환경 조성을 위한 노력을 기울이고 있다. 정신장애인들의 회복 수기에서도 병원과 시설에서 지각한 인권친화적 서비스 환경이 지역사회에서의 활동과 참여를 증진하고, 회복을 촉진하는 과정이 발견된다. 그러나 실제 지각된 인권친화적 서비스 환경과, 활동과 참여, 회복의 관계가 체계적으로 검증된 바가 없다. 이에 따라 본 연구에서는 도나베디안의 구조-과정-결과 이론에 근거하여 지각된 인권친화적 서비스 환경, 활동 및 참여, 회복의 관계를 살펴본다. 이를 위해 2022년 정신장애인 인권친화적 치료환경 구축을 위한 실태조사에 참여한 정신장애인 348명의 의견을 토대로, 구조방정식 및 간접효과 분석을 실시하였다. 분석 결과, 입원병원의 인권친화적 서비스 환경과 지역사회 정신건강시설에서의 인권친화적 서비스 환경(자립지원, 인간존중) 모두 회복에 정적으로 총 효과가 있는 것으로 확인되었다. 활동 및 참여의 매개효과의 경우 지역사회 정신건강시설의 인간존중 환경을 제외하고 모두 완전 혹은 부분 매개효과를 나타냈다. 이러한 연구 결과에 근거하여 정신장애인의 인권친화적 서비스 환경 조성, 회복 증진을 위한 실천적, 정책적 함의를 논하였다.
dc.description.abstractSince a human rights-friendly service environment should be created based on the self-determination of individuals with psychosocial disabilities (IPDs), efforts are being made to establish such environments in inpatient hospitals and community mental health facilities. Recovery memoirs of IPDs indicate that human rights-friendly service environments perceived in hospitals and facilities significantly enhance community engagement and participation, ultimately facilitating recovery. However, the relationship between perceived human rights-friendly service environments, activities and participation, and recovery has not been systematically verified. This study examines the relationship among these factors—perceived human rights-friendly service environments, activities and participation, and recovery—using Donabedian's structure-process-outcome theory. To this end, structural equation modeling, including indirect effect analysis, was conducted using data from 348 IPDs who participated in a 2022 survey on the establishment of human rights-friendly treatment environments. Key findings are as follows: Human rights-friendly service environments in both inpatient hospitals and community mental health facilities (support for independence and respect for human dignity) have a positive total effect on recovery. In the case of the mediating effect of activity and participation, all of them showed complete or partial mediating effects except for the human-respect environment of community mental health facilities. Based on these findings, practical and policy implications for developing human rights-friendly service environments and promoting the recovery of IPDs are discussed.
