Abstract
At the national level, various directions have been proposed for realizing ‘Community Care (Public Hospitals→Community)’. The Seoul Metropolitan Government launched the Seoul Care-City Hospital Health Care Network project to provide continuous and intensive post-discharge health management services for high-risk health groups. This study analyzed the ‘Seoul Care and Seonam Hospital Community Care Model’, using electronic medical records (EMR) of a total of 4,618 patients hospitalized at Seonam Hospital from April 12, 2021 to March 31, 2022. Specifically, the general characteristics of the ‘complex needs group’ with medical and social needs and the results of the evaluation of the discharge requirement of project consent were analyzed. As a result of the analysis, 41 patients of the ‘complex needs group’ who agreed to participate in the project were provided integrated care based on the evaluation of the discharge needs of subjects such as community institution linkage, main program linkage, and community social welfare information.
