dc.description.abstract | 본인부담 상한제는 과도한 의료비로부터 개인과 가구를 보호하는 의료 안전망으로 여러 국가에서 운영하고 있다. 이 연구는 보편적 의료보장 수준이 높은 독일의 본인부담 상한제를 고찰하여, 향후 한국의 본인부담 상한제의 실효성 제고와 의료보험제도 개선을 위한 정책적 시사점을 도출하고자 한다. 독일은 1989년 ‘의료보험개혁법’을 통해 전액 및 일부 본인부담면제, 2003년 ‘의료보험현대화법’ 현재의 본인부담 상한제로 개정되었다. 상한제 운영은 보험자인 질병금고가 상한제 규정 및 원칙에 근거하여 운영한다. 독일에서 환자는 서비스 이용 시점에 10%의 정률부담금을 지불하지만 항목별로 10유로의 고정 금액이 설정되어있고, 가구 단위로 본인부담금이 연간 상한액을 초과하면 면제해주는 이중 안전망으로 설계되어 있다. 본인부담 상한액은 연간 가구 총소득의 2%, 만성질환자 가구는 1%로 낮은 수준이며, 상한액 산정에 ‘가족에 대한 고려’, ‘저소득 가구 특별 규정’, ‘사회부조 한도액 대출’ 등의 특성을 반영하여 차등화하고 있다. 최근 보장 수준의 일부 후퇴에도 불구하고, 소득비례 상한기준 적용을 통한 형평성 제고와 개인 및 가구의 특성을 고려하여, 정교하게 설계된 본인부담 상한제라는 두터운 이중 의료 안전망을 운영함으로써 84.6%의 높은 보장 수준과 2.4%의 낮은 재난적 의료비 발생률을 보이고 있다. 이와 같은 독일 본인부담금 제도의 특징은 유사한 제도를 운영하면서도 독일에 비해 내용과 구조적 한계를 가지고 있는 한국 본인부담제도 개선에 유용하게 활용될 수 있을 것이다. |
dc.description.abstract | Purpose: The aim of this study was to provide policy implications for improving the effectiveness of the policy regarding out-of-pocket maximum in Korea by examining the operational method and characteristics of this policy that is stably implemented in Germany. Coverage: It applies to the benefits of insured persons registered in statutory health insurance. Two-Level Maximum Policy: It sets a maximum cap per item at the time of initial use of the benefit, and establishes the annual cap per household. Management and Operation: The sickness funds operates based on the co-payment ceiling regulations. Ceiling Calculation: The general cap is set at 2% of the annual household income, lowered to 1% for households with chronically ill members. Evaluation: Germany has improved practicality while securing equity between classes by considering household members, economic levels, diseases, and disabilities in co-payment ceiling. Conclusion: Out-of-Pocket Maximum operates as a medical safety net by precisely designing co-payment ceilings based on income. German co-payment system may be utilized for improving co-payment policies in Korea. |
