초록: 본 연구는 2006년과 2009년 사이의 한국복지패널연구조사 자료를 근거하여 4년간의 한국성인의 우울변화양상을 잠재계층성장분석을 통해 파악하였다. 또한, 잠재계층성장모형을 통해 확인된 우울증상의 변화궤적과 흡연, 그리고 음주와의 상관성도 부가적으로 살펴보았다. 분석에 이용된 자료는 총 11,677명의 한국 성인으로, 우울수준에 대한 최소 3년 이상의 자료를 활용할 수 있는 대상자만이 표본에 포함되었다. 우울수준은 11개 항목으로 이루어진 Center for Epidemiologic Studies Depression Scale(CES-D)로 측정되었다. 잠재계층성장분석 결과 세 개의 잠재 계층을 확인하였다:지속적으로 낮은 우울수준 집단(n=9,284, 79.5%), 우울수준의 점차적 완화양상을 띠는 집단(n=1,081, 9.3%), 그리고 지속적으로 상승하는 우울수준 집단(n=1,312, 11.2%). 이렇게 구분된 우울수준 변화궤적과 건강을 해치는 흡연, 그리고 음주행위와의 상관성을 조사하기 위해 이항 로지스틱 회귀분석을 실시한 결과, 총 4년에 걸쳐 지속적으로 낮은 우울수준을 보이는 집단보다 우울수준이 점차 상승한 집단 일수록 흡연(OR=1.90, 95% CI=1.42-2.55)과 음주(OR=1.28, 95% CI=1.04-1.57)를 할 확률이 높은 것으로 나타났다. 본 연구는 장기간에 걸쳐 우울증상의 악화가 예상되는 표적인구를 대상으로 한 적극적인 정신보건복지계획 시행 및 효과적인 복지서비스 전달의 필요성을 지지하는 근거를 마련했다고 볼 수 있다. 또한, 우울증상과 흡연·음주와의 상관성을 보여줌으로써 중독과 동반질환을 통합적으로 개입하는 케어시스템 도입과 아울러 중독전문사회복지사 육성 및 지속적 교육의 필요성을 제안한다.

Using latent class growth modeling(LCGM) and 4-year`s of longitudinal Korean Welfare Panel Study(KWPS, 2006-2009) data, this study examined various trajectories of Korean adults` depressive symptoms and simultaneous smoking and drinking behaviors. Study sample included 11,677 individuals aged 19 years or older at Wave I and those with 3+ years of longitudinal information on respondents` perceived levels of depressive symptoms measured by the 11-item Center for Epidemiologic Studies Depression Scale(CES-D). Data were analyzed via LCGM for classification purposes into subgroups that presented similar change patterns of depressive symptoms over time as varying trajectories. Korean adults` perceived levels of depressive symptoms were classified as the following three trajectories: consistently low(n=9,284, 79.5%), decreasing (n=1,081, 9.3%), and increasing depressive symptoms(n=1,312, 11.2%). Binary logistic regressions were further performed to examine smoking and/or drinking behaviors of each trajectory. Findings showed that Korean adults whose depressive symptoms have increased over time are more likely to smoke(OR=1.90, 95% CI=1.42-2.55) or engage in hazardous/harmful drinking (OR=1.28, 95% CI=1.04-1.57), compared to those whose depression level stayed relatively low over time. Implications include the need to develop and sustain an integrative care system in Korea and to also certify and educate current and prospective mental health practitioners as licensed addiction specialists in tailoring to the needs of Korean mentally ill population.
