의료기술의 혁신과 의료보장체계의 지속성을 위한 국제동향과 정책과제

Abstract

의약기술의 발전은 새로운 질병 치료를 가능하게 하는 신약개발의 성과를 가져오기도 했지만, 새로 도입되는 기술이 반드시 혁신적인 것은 아니며 치료제가 개발되지 않는 미충족 의료 필요의 문제도 크게 해소되지 않고 있다. 한정된 재정으로 운영되는 의료보장체계에서는 나날이 고가화되는 의료 기술을 효과적으로 급여하는 것이 중요한 과제가 되고 있다. 의료기술의 개발과 사회경제적 조건의 부조화를 최소화하기 위해서는 기술의 개발 단계에서 사회적 필요와 가치를 반영할 수 있는 기전이 마련되고, 연구개발 과정의 위험과 성과가 사회에서 공유되어 최종 산물에 대한 접근성이 확보되어야 한다.

의료보장 재정은 무한히 늘릴 수 있는 것이 아니며 건강문제 또한 의료기술로 모두 해결할 수는 없다. 의약기술 발전의 결실을 질병치료와 건강문제의 해결에 지혜롭게 활용하기 위해서는 가용한 재원을 어떤 의료기술에 언제 얼마나 지불할 지에 대하여 신뢰 가능한 정보를 바탕으로 사회 전체적으로 논의, 합의하는 구조가 필요하다.

Today we are seeing increasing number of high-cost healthcare technologies. However, only a small number of them are classified as innovative and many countries are struggling to effectively fund them. Current incentive system based on intellectual property rights for research and development has been criticised for the profit-maximizing high price and the subsequent restricted accessibility to healthcare technology. In this respect, several new innovation models have been suggested and discussed for stimulating healthcare technology development addressing unmet health needs and ensuring accessibility to the technology. The low level of innovation of most new medicines has led to more critical assessment of their value in many health care systems. Given the soaring number of high-cost healthcare technology adoptions, simply continuing to react case-by-case is not sustainable. We need to be more forward-thinking and to pursue policy measures to balance the value of innovation with equitable, affordable patient access.
