dc.description.abstract대부분의 국가에서 공공사회복지지출은 노령(Old Age)과 보건(Health)부문의 비중이 가장 높게 나타난다. 우리나라 역시 최근 보건부문의 지출 증가가 매우 급격하게 나타나고 있다. 그러나 지금까지 복지개혁은 주로 노령부문을 중심으로 한 연금 개혁과 서비스 전달체계를 중심으로 이루어진 반면 최근에는 보건의료개혁에 대한 욕구가 높아진 것이다. 독일과 네덜란드는 개혁의 배경과 경로는 달랐지만, 보험자와 공급자의 합리적인 의사결정방식과 사회연대의식에 기반을 두고 오랜 기간 개혁방안을 모색하였다는 공통점이 있다. 그러나 우리나라는 보건의료개혁에 대한 청사진이 뚜렷하게 제시되지 못하고 있다는 점에서 차이가 있다. 이러한 이유 중 하나는 국가 주도로 건강보험이 도입되고 확대되었으나 민간 중심으로 의료공급이 확대되었기 때문이다. 대부분의 사회서비스가 그렇듯이 우리나라는 "정부 주도의 도입, 민간 주도의 공급"이라는 모순을 안고 제도가 형성되었으며, 정부와 밀접한 관련이 있는 단일 보험자와 보건의료서비스의 공급에 주도적인 역할을 담당하는 민간 공급자간 동등한 합의와 조정에 대한 프로세스가 발달하지 못하였다. 이제 사회연대의식에 기반을 두고 보건개혁에 대한 논의를 시작할 때다.
