| dc.description.abstract | 최근에 제안된 DRG 지불제도의 모형을 소개하고 동 DRG 모형개발에 영향을 미친 일본의 DPC 지불제도와 비교하면서 평가를 하였다. 특히 의료기관의 자율적인 선택제가 갖는 왜곡, 일당지불에 기반하는 DRG 수가가 가져다줄 비용전가의 문제 등을 조명하였다. 그리고 DRG 지불제도의 향후 정책방향은 당초 본 제도가 지향하는 목적과 원칙에 충실하도록 발전시켜야 하며, DRG 지불제도가 소비자-공급자-보험자 3자 모두에게 이득이 되는 방안을 모색하였다. 그리고 현행 행위별수가제 하에서 의료자원의 이용실태에 대한 정확한 진단이 우선 내려져야 할 것을 강조하였다. 궁극적으로 상대가치 행위수가제나 DRG 지불제도, 주치의방식의 인두제, 총액계약제 등이 지향하는 목표는 같기 때문에 소비자-공급자-보험자 3자 모두 승자가 되는 길에 동참할 것을 제안한다. |
