한국의 담배규제정책 관련 근거중심 문헌고찰 및 평가: 젊은 성인을 위한 담배규제정책을 중심으로

Abstract

최근 국내 보건의료가 선진화됨에 따라 객관적 근거에 기반하여 의사결정을 내리는 근거기반정책에 대한 필요성과 관심이 증가하고 있다. 이에 본 연구에서는 사회경제학적으로 상당한 비용을 발생시키고, 보건학적으로 중요성이 매우 큰 담배규제정책 중 흡연에 대한 위험에 많이 노출되어 있지만 상대적으로 우선순위가 후순위에 있는 젊은 성인을 대상으로 한 담배규제정책에 대하여 근거중심 고찰을 진행하였다. 연구결과 담배규제정책과 관련된 효과성에 관한 연구는 정책별로 편중되는 경향을 보였으며 여성의 흡연에 대한 정책 및 연구 또한 부족한 것으로 나타났다. 정책별 근거수준에 대한 평가를 살펴보면, 담뱃세 인상과 같은 가격정책의 경우 상대적으로 높은 수준의 근거수준을 보였으나 전반적인 담배규제정책은 낮은 수준의 근거수준을 보였다. 본 연구의 결과는 장/단기적으로 젊은 성인의 금연에 유의한 효과를 나타낸 담뱃세 인상 정책을 체계적으로 시행하고, 젊은 여성 흡연자에 대한 심층적 연구를 진행하며 새로운 담배시장의 흐름에 맞는 적절한 연구 및 선제적 정책 시행이 검토되어야 할 필요가 있음을 제안한다.

The need for and interest in evidence-based health policy-making have increased with the recent development in health care in Korea. Therefore, this research conducts evidence-based evaluation of tobacco control policy for young adults who are highly exposed to the danger of smoking, but rank behind in the priority list among anti-smoking policies that incur significant socioeconomic costs and are of significant importance in public health. Research findings show that research on effectiveness of anti-smoking policies tends to exhibit a skewed distribution depending on policies and lacks policies and research on female smoking. In terms of evaluation of the level of evidence depending on policies, the results show that while research on price policies such as price increase in tobacco exhibits a relatively higher level of evidence than research on non-price policies, anti-smoking policies generally show a low level of evidence. This research proposes the need for systematic operation of policy on tobacco price increase that has shown a meaningful effect on anti-smoking for young adults in both short and long terms, an indepth study of young female smokers, and appropriate research and proactive policy operation in accordance with the new trends in the tobacco industry.
