노인 대상 한국어판 자가보고 복약이행도 측정도구(ARMS-K)의 신뢰도 및 타당도 검증

Abstract

본 연구의 목적은 한국노인의 복약이행도를 사정하기 위해 한국판 Adherence of Refills and Medications Scale (ARMS)의 신뢰도와 타당도를 검증하는 것이다. 본 연구를 위해 N시와 C 지역의 당뇨병과 고혈압을 앓고 있는 242명의 노인이 참여하였다. IBM SPSS 22.0 프로그램을 사용하여 신뢰도와 구성타당도가 측정되었다. 구성타당도 검증을 위해 개별 문항과 전체 문항과의 상관관계 및 탐색적 요인분석이 실시되었다. 한 문항이 다른 문항들과의 상관성이 낮아 제거되었다. 주요인분석을 통해 도출된 3개 요인이 64.3%를 설명하였다. 3가지 하위요인은 복약 순응의지, 재처방 순응의지, 처방용량 순응의지로 명명되었다. 내적일관성은 신뢰도 Cronbach’s a .83에 의해 지지되었다. 본 연구의 결과는 원 ARMS 도구와 비교하여 다른 요인 구조를 보여주었고, 한국판 ARMS는 만족할만한 신뢰도와 타당도를 보여주어 향후에 사용이 가능할 것이다.

The purpose of this study was to test the validity and reliability of the Korean version of Adherence of Refills and Medications Scale (ARMS-K) to assess medication adherence in the Korean elderly. The ARMS-K was developed through forward- backward translation technic. A total of 242 elderly individuals with diabetes and hypertension participated to test ARMS-K in N city and C area. Internal consistency reliability and construct validity were calculated using IBM SPSS Statistics 22.0 program. Item-to-total correlations and exploratory factor analysis were conducted to test construct validity. One item was removed because of its low correlation with other items. Through principal component analysis, three factors explained 64.3% of the variance. Three subscales were named as taking medication as prescribed, refilling medications on schedule and medication misuse. Internal consistency was supported by Cronbach’s a=.83. The results of this study showed different factor structures compared to those of original ARMS. The Korean ARMS-K showed satisfactory construct validity and reliability.
