dc.description.abstract	미국은 메디케이드(Medicaid)를 중심축으로 연방정부와 각 개별 주가 자폐성장애인에 대해 보건-의료-교육-고용-복지서비스를 통합적으로 그리고 각 주의 상황에 맞게 독립적으로 제공하도록 지원하고 있다. 이를 위해 메디케이드는 자폐성장애로 인한 중산층 등의 가계 파산을 막기 위해 (1) 1915(c) 가정·지역사회 기반 서비스(HCBS: Home & Community-Based Services) 면제(Waiver) 규정을 이용하여 메디케이드 수급 범위를 확대하고, (2) 연방 메디케이드 차원에서 급여 범위를 지속적으로 확장하며, (3) 1915(i) 가정·지역사회 기반 서비스 조항을 통해 개별 주의 특성에 맞는 자폐성장애 특별 프로그램을 인정해 주는 정책을 시행하고 있다. 이와 같은 미국의 메디케이드 역할 확대는 자폐성장애인을 위한 통합적이고 포괄적인 서비스를 구축해야 하는 우리에게 많은 시사점을 제공한다.
