체질량지수와 비만이 개인의료비지출에 미치는 영향에 대한 분석: Control Function 방법

Abstract

본 연구에서는 관찰되지 않는 개인적인 이질성의 통제를 위한 control function 방법과 의료비지출변수의 특징을 잘 고려하는 two part 모형을 이용하여 우리나라에서 체질량지수와 비만상태가 개인의료비지출에 미치는 영향을 한국의료패널 2009-2015년간 데이터를 이용하여 분석하였다. 연구결과 전체표본에서 체질량지수 한 단위의 증가는 약 5만 원의 추가적인 연간의료비지출을, 비만상태는 비만이 아닌 상태와 비교했을 때 92만 원의 추가적인 연간의료비지출을 유발하는 것으로 나타났다. 더 나아가 분위수회귀의 결과를 볼 때 체질량지수와 비만상태는 의료비지출액분포의 상단으로 올라갈수록, 즉 건강이 좋지 않아 의료비지출이 큰 집단에 더 큰 영향을 미치는 것으로 나타난다. 또한 대부분의 결과에서 control function 변수에 대한 추정계수가 유의하게 음의 값을 가지며, 이는 내생성이 적절하게 통제되지 않는 경우 과소추정이 발생할 수 있음을 보여준다.

This study analyzed the effects of body mass index (BMI) and obesity status on individual medical expenditures in Korea, using the control function method to control for unobserved individual heterogeneity and the two-part model to consider the characteristics of medical expenditure variables. For this purpose, the Korea Heath Panel (2009-2015) is utilized. The study found that an increase in one unit of BMI in the overall sample resulted in an additional annual medical expenditures of about 50,000 won, while being -obese, compared to being non-obese, caused an additional annual medical expenditure of 920,000 won. Furthermore, the results of the quantile regression show that BMI and being -obese have a greater effect on the upper tail of the health expenditure distribution. Also, in most of the results, the estimation coefficients for the control function variable are significantly negative, indicating that underestimation may occur if the endogeneity is not properly controlled.
