dc.description.abstract본 연구의 목적은 1차년도 서울시 ‘찾아가는 동주민센터’사업 중 65세, 70세 도래 노인 대상 전수방문과 대상자의 건강 수준과 필요에 따른 후속 서비스를 제공하는 보편 방문건강관리사업이 사업대상자들의 건강에 미친 영향을 이중차이기법을 적용하여 평가하는 것이다. 분석을 위해 지역사회건강조사 응답자 중 서울시 거주 65세, 70세 도래 노인 총 5,273명(사업 시행동 주민 1,215명과 사업 미시행동 주민 4,058명)이 최종 분석에 포함되었다. 2014~2016년 지역사회건강조사의 서울시 자료를 바탕으로 매년 산출 가능한 건강 수준과 만성질환 관리, 정신건강, 건강 행동, 보건의료 서비스 이용 영역의 건강 결과지표를 선정, 1차년도 사업에 대한 평가를 실시하였다. 사업 시행 전후 시행동과 미시행동 주민들의 건강 결과 차이에 대한 분석 결과, 미시행동 대비 시행동의 65세, 70세 도래 노인집단에서 현재 흡연율 감소(13.2%에서 8.2%, 미시행동 대비 5.7%p 감소, p=0.042)가 나타났다. 아울러 미시행동 대비 시행동의 65세, 70세 도래 독거노인 집단에서는 저염식 실천율 향상(15.7%에서 27.1%, 미시행동 대비 22.7%p 증가, p=0.006)의 결과를 확인할 수 있었다. 이와 같이 엄밀한 준실험적 설계방법을 적용하여 확인된 사업 시행 초기의 건강 효과는 제한적인 것으로 나타났다. 보다 지속가능하고 포괄적인 지역사회 보건복지 서비스 전달체계 구축을 위하여 사업 안정기 이후의 자료를 활용한 추가적인 효과평가와 더불어 후속 연구에 근거한 환류체계 마련이 필요할 것이다.
dc.description.abstractThe purpose of this study was to evaluate the health impact of the 1st year (2015)’s visiting health services for the elderly, a program of the outreach community center initiative in Seoul by applying Difference in Difference (DID) technique. For the analysis of DID technique, 5,273 residents of 65 years old and 70 years old dwellers of Seoul who responded to 2014-2016 Community Health Survey (1,215 residents in the participating districts and 4,058 residents in the non-participating districts) were included in the final analytic model. We selected annual health indices, health outcomes indicators for chronic diseases, mental health, health behaviors, and health care service utilization as the outcomes of the initiative. The DID analyses revealed that the current smoking rate reduced (13.2% to 8.2%, -5.7%p, p=0.042) among 65 and 70 year olds living in the participating districts compared to their counterparts living in the non-participating districts. Subgroup analysis among the elderly living alone in the participating districts showed improvement of low-salt intake (15.7% to 27.1%, +22.7%p, p=0.006). This study showed that the health impact was limited partly due to the early stage of the project implementation. Further study is required to implement a more sustainable and comprehensive community health-welfare delivery system and to develop effective feedback schemes.
