일반 성인의 건강문해력의 하위 차원과 건강 관련 행위간의 관계

Abstract

건강정보 과잉시대에 건강문해력(Health Literacy)은 개인이 건강관리를 위해 갖추어야 할 능력 중 하나이다. 건강문해력을 일차원적으로 접근했던 기존 연구와 달리, 본 연구는 건강문해력의 다차원적 개념에 주목, 각각의 차원이 의료서비스 이용자의 다양한 건강 관련 행위에 어떠한 영향을 미치는지를 탐색하고자 하였다. 구체적으로 건강문해력의 네 가지 역량 차원(건강정보에 대한 접근, 이해, 판별, 활용 능력)이 건강 관련 행위(인터넷 건강정보 이용, 건강관리 행위, 의사-환자 커뮤니케이션 효능감, 닥터 쇼핑)에 어떻게 영향을 미치는지 살펴보았다. 전국 성인남녀(N=685)를 대상으로 실시한 온라인 설문조사를 분석한 결과, 접근 역량은 인터넷 건강정보 이용과, 활용 역량은 건강관리 행위와, 판별 역량은 닥터쇼핑과 정적 관계를 보였다. 의사-환자 커뮤니케이션 효능감의 경우 이해 및 활용 능력 모두와 유의미한 정적 관계를 보였다. 이러한 본 연구의 결과는 개별 건강 관련 행위와 건강문해력 역량 차원 간의 관계를 밝힘으로써 건강문해력 교육을 위한 전략적 지침을 제공한다.

In the age of abundant health information, health literacy has become one of the required skills to manage one’s own health. While previous studies have often taken a unidimensional approach to health literacy, this study focused on the multi-dimensionality of health literacy and its relationships with various health-related behaviors. Specifically, this study explored how the different sub-dimensions of health literacy (i.e., access, understanding, appraisal, and application) relate with health information usage on the Internet, health management behaviors, doctor-patient communication efficacy, and doctor shopping. Our analysis of an online survey of 685 Korean adults revealed that each sub-dimension of health literacy uniquely relates to health-related behaviors: access capacity was positively related with health information usage on the Internet; understanding capacity was positively related with doctor-patient communication; appraisal capacity was positively related with doctor shopping; application capacity was positively related with health management behaviors and doctor-patient communication. By identifying specific types of health literacy that are relevant to a particular health behavior, the present study provides strategic guidance for health promotion and health literacy education.
