영국의 치매대응 정책분석과 시사점: 스코틀랜드의 국가치매전략을 중심으로

Abstract

본 연구는 영국 스코틀랜드의 국가치매전략 정책을 분석하고 이를 기반으로 한국의 치매국가책임제에 대한 정책적 방향성과 시사점을 돌출하는데 목적을 두고 있다. 이를 위해 치매에 대한 관점인 의료적 모델, 사회적 모델, 인간중심케어 모델을 분석틀로 사용하여 스코틀랜드의 국가치매전략를 분석하고 이를 평가하였다. 그 결과 스코틀랜드의 국가치매전략에 핵심은 치매경로에 따른 개별화된 맞춤형 지원으로 진단직후 개입을 위한 사후치매진단지원과 생애말기케어를 강화하고 있다. 일련의 모든 과정에 인간중심케어가 중심이 되고 있다. 인간중심케어 모델이 실제로 구현될 수 있었던 것은 첫째, 정책의 철학적 논의가 활성화 되어 있고, 둘째, 복지국가라는 제도적 유산이 존재하며, 셋째, 당사자가 정책의 주체였기 때문이다. 즉, 치매정책의 형성과 이행, 모니터링에 치매인 당사자와 시민단체가 정부와 협업을 하고 있기 때문에 가능했다. 한국의 정책적 시사점은 다음과 같다. 첫째, 치매정책의 철학적 고찰이 필요하다. 둘째, 보편적 복지의 확대와 함께 인간중심케어 모델을 구축해 가야한다. 셋째, 당사자가 정책의 주체가 될 수 있도록 실질적 거버넌스를 구축해야 한다.

The purpose of this study is to analyze the policy of Scotland’s National Dementia Strategy. It aims to provide policy direction and implications for Korea’s dementia national responsibility system. For this purpose, this study examined Scotland’s National Dementia Strategy according to the medical model, social model, and person-centered care model, which are distinct viewpoints of dementia. As a result, the core of Scotland’s National Dementia Strategy is person-centered approach from the diagnosis dementia to end of life. The person-centered care model is found to be an essential component in the implementation of the strategy. The reason why the strategy has been developed in Scotland is base on the philosophical discussion, the institutional legacy of welfare state, and the civic society and people with dementia who are participated in the making of the strategy. The policy implications of Korea are as follows. Firstly, philosophical consideration of dementia policy is needed. Secondly, institutional legacy of universal welfare should be created. The last but not least, substantial governance should be established so that the parties can become the subject of the policy.
