아동 성폭력 피해 경험이 자살 행동 유형에미치는 영향: 잠재계층분석을 중심으로

Abstract

본 연구는 자살 행동(자살 생각, 계획, 시도, 치료)의 잠재계층을 구분하고, 아동 성폭력 피해 경험이 자살 행동 유형에 어떠한 영향을 미치는지 살펴보고자 하였다. 이를 위해 청소년건강행태온라인조사 8차년도 자료를 사용하였으며, 중1~고3 74,186명을 대상으로 잠재계층분석과 다항로지스틱 회귀분석을 실시하였다. 본 연구의 결과는 다음과 같다. 첫째, 자살 행동은 3개의 잠재계층으로 구분되었으며, 이는 “자살 저위험형(91.4%)”, “자살 사고형(7.4%)”, “자살 실행위험형(1.2%)”으로 명명하였다. 둘째, 성폭력 피해 아동은 “자살 저위험형”에 비해 “자살 사고형”에 속할 가능성이 더 높았다. 그리고 “자살 실행위험형”에 속할 가능성이 더 높았다. 이러한 결과를 바탕으로 교사 및 학부모 교육을 통해 아동 성폭력 피해 경험이 자살 행동의 중요한 위험 요소임을 인식시켜야 하며 자살 예방을 위해 아동 성폭력 피해자 개입 및 치료와 아동 성폭력 전담 기관과 자살예방센터, 정신건강복지센터 등의 연계체계 구축이 필요함을 제언하였다.

This study identified the patterns of suicide behavior (suicide ideation, plan, attempt, treatment) and examined the effect of the experience of child sexual violence on patterns of suicide behavior. We analyzed the data from the 8th year Korea Youth Risk Behavior Web-Based Survey, including 74,186 students in seventh to twelve graders. Latent class analysis and multi-logistic regression models were used for the data analysis. The results of this study were as follows. First, suicide behavior patterns were classified into three latent groups: Low suicide risk group (91.4%); Suicide ideation group (7.4%); Suicide attempt risk group (1.2%). Second, the children as victims of sexual violence were more likely to belong to the “Suicide attempt risk group” than the “Low suicide risk group”. And it was more likely to belong to “Suicide thoughts group” than the “Low suicide risk group”. Findings suggested that it should be recognized through teacher and parental education that the experience of child sexual abuse is an important risk factor for suicide behavior. In order to prevent suicide, intervention and treatment are needed for victims of child sexual violence. In addition, it is suggested that a linkage should be established between services on child sexual violence, suicide prevention centers and mental health welfare centers.
