간호사와 간호조무사의 업무 및 자격관리에 대한 정책결정과정 연구: 킹던의 정책흐름모형을 중심으로

Abstract

2015년 12월, 의료법 개정을 통해 간호사 업무규정이 구체적으로 명시되었고 간호조무사 자격관리가 강화되었으며, 간호사에게 간호조무사에 대한 관리감독권한을 부여함으로써 간호인력 간의 업무가 명확히 구분되었다. 본 연구에서는 이와 같은 정책결정과정을 Kingdon(1984)이 제시한 정책흐름모형의 주요 요소별로 분석하였다. 문제의 흐름으로는 간호사와 간호조무사의 상이한 관리체계 및 불명확한 업무경계, 간호사 수급 불균형에 의한 간호조무사 활동 급증, 간호조무사의 자격관리 미비 및 질 관리 필요 대두 등을 도출하였다. 정치의 흐름으로는 기존 정권의 관련 정책 추진의 맥락과 국회 입법절차 과정 등에 대해 분석하였다. 정책대안의 흐름으로는 간호조무사 자격사항에 대한 협의, 보건의료직능발전위원회 논의를 포함한 정부의 보건의료정책 추진 방향과의 부합, 간호인력개편 1･2차 협의체 운영을 통한 직능단체의 의견수렴 등을 제시하였다. 정책선도자에는 간호계의 정치적 영향력 향상, 대한간호협회의 지원, 간호사 출신 국회의원의 리더십 발휘를 꼽았다. 정책의 창으로 간호간병통합서비스제도 및 메르스 사태와 법안심사소위의 논의 의결과정을 기술하였으며 최종적인 정책 산출로서 간호인력 관련 의료법 개정에 대해 도출하였다. 문제‧정치‧정책대안이 만나는 지점에서 메르스 사태와 간호간병통합서비스제도로 인해 정책의제화가 가능하였고, 법안심사소위 논의 의결과정에서 간호사 출신 국회의원이 정책선도자로서 협의를 이끌어내어 정책이 창이 열렸으며 궁극적으로 의료법 개정이라는 정책결정이 이루어지게 되었다.

Amendment of Medical Services Act regarding Nursing Professions was done on December 2015. This study aims to analyze and assess policy making process on nursing service and qualification management between nurse and nurse assistant using Policy Stream Model by Kingdon in 1984. As problem stream, heterogeneity of management system between two nursing staffs, increase of nurse assistant's activity due to disparity between supply and demand, and needs for quality control for nurse assistants were raised. Political steam was continuos policy presentation by health authority, and legislative procedure within National Assembly. With respect to policy stream, discussions on qualification of nurse assistant, conformity of direction by health authority and acceptance of opinion from several national associations on health were produced. Policy entrepreneur was improvement of nursing politician and nursing leadership supported by Korea Nurses Association, and nurse politician's active role on revision of law. As policy window, policy for integrated nursing and caring service as well as MERS-CoV explosion, and judging bill process were reviewed. Nursing politician was playing a significant role for opening policy window which finally leads to policy decision.
