초록: 본 연구의 목적은 한국어판 노인요양시설입소 동의능력사정도구(K-CMAD)의 신뢰도와 타당도를 검증하는 것이다. 이를 위해서 노인요양시설에 거주하고 있는 289명의 노인을 대상으로 일반적 특성, 시설입소 고지된 동의 및 관련 변수 등에 대해 설문조사 하였다. 기술적 통계, 신뢰도 분석, 확인적 요인분석(CFA), 상관관계분석, 라쉬모형분석을 실시하였으며, SPSS 21.0, Mplus 5.2, 그리고 WINSTEPS 3.81.0 버전을 활용하였다. 분석결과 동의능력사정도구(K-CMAD)의 신뢰도 계수는 .869로 내적 일관성이 높은 것으로 나타났으며, 확인적 요인분석 결과에서 모델 적합도 지수(RMSEA = .042, CFI= .999, TLI= .998, WRMR= .418) 모두 양호한 것으로 나타나 구성타당도를 검증하였다. 수렴타당도는 동일개념을 측정하는 동의능력평가자 보조설문과의 높은 상관관계(r = .822, p < .001), 그리고 이론적으로 관련성이 있는 변수로서 일상생활능력(r= -.254, p <.001) 및 인지건강(r= .536, p <.001)과 유의한 상관관계로 성립되었다. 끝으로 라쉬모형 분석 결과, 시설입소 동의능력사정도구의 5개 문항에 대한 내적합도 및 외적합도가 모두 양호한 수준이었지만, 난이도가 높은 문항을 추가하는 것과 3점 척도에서 2점 척도로 응답범주를 재범주화할 필요성이 제기되었다. 본 척도(K-CMAD)는 노인요양시설의 입소상담과정에서 시설입소를 희망하는 노인의 동의능력을 측정할 수 있는 도구로 활용될 수 있을 것으로 기대된다.

This study evaluated the reliability and validity of the K-CMAD to nursing homes. Data were taken from survey responses from 289 nursing home residents who were asked about the demographic characteristics, capacity to consent to nursing home admission, and variables related with decision making capacity. Using SPSS 21.0, Mplus 5.2 and WINSTEPS 3.81.0, descriptive analyses, internal reliability analyses, confirmatory factor analyses, correlation analyses, and Rasch Model analyses were conducted in this study. The results showed that the internal consistency of the K-CMAD was good(.869) and the construct validity was acceptable with the good levels of the overall model fit(RMSEA= .042, CFI= .999, TLI= .998, WRMR= .418). The claims for the convergent validity would be supported because ADL(r= -.254, p< .001), cognitive disabilities(r= .536, p< .001), and Evaluator Questionnaire Assist(r= .822, p< .001) were statistically associated with the K-CMAD. Lastly, the Rasch model analyses indicated that the five items of the scale had acceptable levels of infit and outfit, and a dichotomous scale would be more appropriate than the three pointed response options. The K-CMAD can be used for grontological social workers to screen whether an individual is able to provide consent for admission to nursing homes.
